He was loved and cherished by many people including : his parents, Helen Kostashuk (Yelena) and Dymyto Kostashuk (Dan); his wife Marianne; his children, Cathy Haub (Rod) of Athabasca and Jack Kostashuk of Athabasca; and his grandchildren, Melanie, Tyler, Dustin, Pamella, Katlyn and Cole. He was also cherished by numerous grandchildren and great grandchildren and other family members.

In lieu of flowers, anyone who wishes to do so can make a memorial donation in Jack's name to the school nutrition program.
