He was predeceased by : his father Wladyslaw Kozak; his brother Tadeusz Kozak; and his grandmother Bronislawa Piech. He is survived by : his mother Helena Kozak; his brother John Kozak (Mary); his sister-in-law Renata Kozak; his nephews, Tomasz Kozak (Maria Yahnee Cabagui), Peter Kozak and Pawel Kozak; and his niece Agnieszka Kozak.
