He was predeceased by: his parents, Peggy Cooke and Gerald Cooke; his niece Allison Melvin; and his great grandchild Denis Paul. He is survived by: his wife Paula; his son Christopher (Laura); his grandchildren, Taylor, Madison and Cailyn; his siblings, Shirley Melvin (Patrick), Mary Cooke, Emmanuel Sr. (Num), Tom Sr. (Sheila) and Aunt Sr.; his mother-in-law Madeline Andrews; and his siblings-in-law, Christine Bradshaw, Tina Quinn (James) and Mark Andrews.

In lieu of flowers, donations are gratefully accepted to the Tom Baker Cancer Centre in memory of Denis.
