He was predeceased by : his wife Patti Hoffman; his son Dustin Hoffman; his parents, Edward Hoffman and Violet Hoffman; and his sisters, Lynne Janke and Judy Petschulat.

He is survived by : his son Jon Hoffman (Danielle Ostifichuk); his siblings, Melody Maher, Lyle Hoffman and Leonard Hoffman (Lynn); his mother-in-law Betty Jacobson; and his sister-in-law Barb Jacobson.
