He is survived by: his parents, Michael C. Kuzminski and Alanna M. Krook; his siblings, Kassandra L. Krook, Eleanor C. Kuzminski, Carl J. Kuzminski and Graham L. Kuzminski; his nephews, Damian and Jayden; and his niece Ayla.

In lieu of flowers, any donations may be made to drug prevention and rehabilitation, or to the LBGTQ Community and would be gratefully appreciated.
