She was predeceased by: her parent Ellis Forest; and her brother Blair Forest. She is survived by: her husband Rick Provencher; her children, Kayla, Troy and Mya; and her grandchildren, Kalyna, Anastazia, Mila, Brantley, Jackson, Yara and Cecilia.
