He was predeceased by : his brother Joshua Collins; and his cousins, Bradley Hamelin, Buck McConnell, Gayle Collins, Carmen Dee Cardinal and Denise Collins.

He is survived by : his wife Nicole Lavallee; his children, Lennox, Lilyanne, Natalia, Gage and Grady; his siblings, Matthew Collins, Jeremy Collins, Justin Collins, Casey Collins, Randie Collins, Jamie Cardinal, Clayton Lewis, Shelley-Lynn Lewis and Kyle Lewis; his parents, James Collins and Myrtle Lewis; and his father-in-law Ron Lavallee. He is also survived by numerous aunts, uncles, nieces, nephews, cousins & friends.
