He was predeceased by: his wife Catharina. He is survived by: his spouse Tiny; his son-in-law Mike; his children, Leo Weyenberg, Miriam Weyenberg and Dorothy Johansen (Mike); his granddaughter Jamie Johansen; his brother Andre Weijenberg (Bertie); and also Sandra Quadakkers.
In lieu of flowers, memorial donations may be made in Jack's name to St. Michael's Church Building Fund.
