She was predeceased by: her parents, Walter Hyde and Mary Hyde; her parents-in-law, Anton and Lydia Form; her sister-in-law Fran Sawchyn (Gary Form); and her grandson Tanner Husch.

She is survived by: her husband Alvin Form; her children, Chelsa Husch (Darryl Graham) and Bradley Form; her grandson Brendon Husch; her great grandchild Zoey; her grandson Tyler Husch (Vikki); her great grandchildren, Noah, Nate and Layla; her grandchildren, Ashten Form and Jesse Form; and her sister Barbra Hawkins (Les).

Donations in memory of Heather may be made to the Paradise Hill Care Home or to donor's charity of choice.
