He was loved and cherished by many people including : his wife Gail; his children, Leeanne (Con), Dean (Tanya) and Darla (Trevor); his grandchildren, Maverick (Jessie), Jordanna (Rylan), Jantzen (Hayley), Drayce, Ricayla (Ben), Mackenna (Dawson), Jayven, Darby (Dawson) and Mazden; and his great grandchildren, Sophia, Aubrie and Porter. He was also cherished by numerous extended family and close friends.

Donations in memory of Jack may be made to Lloydminster Regional Health Foundation (Hospital) or Stollery Children's Hospital.

Now it is time for him to rest. I know how hard it is to let a loved one leave us. Cherish all the memories you have.
