She was predeceased by : her father Gerard Van Dyk. She is survived by : Her loving partner, Greg White, her children, Aimee Donovan (Conal) and Trevor Sackman (Alysha); her grandchildren, Zoey, Lara, Alexander, Larkin and Isla; her mother Adriana Van Dyk; and her siblings, Mary Rose, Elfriede Hoose (Don), Adriana MacKay (James) and Jerry Van Dyk. She is also survived by numerous nephews, nieces, and cousins.

In lieu of flowers, donations in Jan's memory may be made to the Margery E. Yuill Cancer Centre c/o Medicine Hat & District Health Foundation, 666 - 5th Street SW, Medicine Hat, Alberta T1A 4H6.
