He was predeceased by: his parents, Walter Czuboka and Mabel Czuboka. He is survived by: his wife June; his children, Denise (Andy), Bonnie (Chris) and Mike (Chelsey); his grandchildren, Paige (Sheldon), Conner (Courtney), Ethan, Carter, Ava and Ryan; and his sisters, Susan (Wayne) and Shirley (Doug). He is also survived by loving extended family members and friends.
