He is survived by: his children, Adam of London, ON, Simon of Kelowna, BC and Leah of Lake Country, BC.; the mother of his children Shannon Kuiack (Reid) of London, ON.; his parents, Max Kuiack of London, ON and Rita Kuiack (Mask) of Ottawa, ON; his sisters, Denise (Kevin Horoky) of London, ON, Shelley (John Wendell) of Ottawa, ON, Joanne (Philip Brown) of Strathroy, ON, Stephanie (Howard Jones) of Mississauga, ON, Montego Bay and Jamaica; and his significant other Beth Flynn (Burton). He is also survived by aunts, uncles, nieces, nephews and cousins.

Due to COVID-19 restrictions, there will be no service at this time.
