He was predeceased by: his parents, Helen Kozak and George Kozak. He is survived by: his brothers, Paul Kozak (Wendy) of White Rock and Ralph Kozak (late Kim) of South Carolina; his niece Kayla Kozak (Arizona); and his goddaughter Katie Yeomans (Windsor).

“I have just heard the news and am So sad. I knew Keith from point grey golf club. He was a wonderful guy who made us all laugh.. the last time I saw him he even suggested a special Japanese gin that I should try. Every time I have it I think of him. I was really sorry when he left here. He was always laughing... even taught us how we should cook salmon. I am so sorry for him and for the loss everyone will feel that's he's no longer here.”
