He was predeceased by: his parents, Mary Kowalchuk and Steve Kowalchuk. He is survived by: his wife Sandra; his daughter Allison; his siblings, Janet (Carl) and Ron (Karen); and his siblings-in-law, Ian (Tanis), Andrew and Karen (Len). He is also survived by many nieces and nephews.
The family is grateful for the support and resources provided by U-Turn Parkinson's and the Movement Disorder Clinic, and in lieu of flowers, donations may be made to help continue their work.
