He is survived by: his parents, Donald Cook and Doris Cook (Strugnell); his wife Debbie; his daughter Amanda Cook (Kian Hutchinson); his stepchildren, Amanda Coy (Tim), Selina Banks, Ross Banks (Jodell Cameron) and Rico Fink; his sisters, Susan Macek (Mike) and Sherry Elliott (Andrew); and his nephews, Justin and James Macek.
