He is survived by: his wife Donna; his children, Steven (Roberth), Susan (James) and Scott (Codi); his grandchildren, Layla, Jack and Scarlett; his siblings, Derek (Dolly), Ralph Alan (Ann) and Carol (Gerard); his in-laws, Gord Forward and Linda Moore; his mother-in-law Jean Forward; his nieces and nephews, Ayla, Mark, Joey, Keven, Lydia, Natalie, Michelle, Caroline, Kasandra, Michael, Perry, Keith, Chad, Corey, Colin, Trevor and Adam; his great-nieces and great-nephews, Luke, Jake, Ava, Penny, Alice and Romi; his close friends, Jim, Rhonda Marshall, Bryan and Colleen Murphy; and his caregiver Dave.
