He was predeceased by : his parents, John Maifeld and Eileen Maifeld (Dillon); and his sister Marlene. He is survived by : his wife Corrine Langley Mayfield; his son Scott Maifeld; his stepchildren, Jeff Simpson of London, UK, Vicky Simpson of Fredericton, NB, Matthew Simpson of New Westminster, BC and Kristy Simpson of Kentville, NS.; and his friend Sandra Weir.

Donations in memory of Jack Mayfield may be made to the Canadian Cancer Society.
