He is survived by: his father Kenneth Fredericks; his significant other Sandi; his daughter Jaclyn; his grandson Mathew; his daughter Tammy Bona; his siblings, Michael "Mike" (Melissa), Jim (Deanna), Pauline (Pat) and Dianne (late David); and his caregiver Emily.
