He was predeceased by: his father Donald Jack; and his brother Bruce Jack. He is survived by: his mother Connie Jack (Rudolph); his significant other Gweneth Boutilier; his children, Kurt Schanz-Jack (Randi-Lee) and Rebecca Roso (Matt) of Monson, MA; his siblings, Darla Porter-Jack (Ricky) and Sandy Jack (Peggy); his sister-in-law Rhonda Jack; and his grandchildren, Audrey-Lee Schanz-Jack and Nathan Kmieczak. He is also survived by many nieces, nephews.
