He was predeceased by: his parents, Frank Cook and Belle Cook; and his wife Dorothy. He is survived by: his children, Michael (Janice) and Sally Jo (Philip); his close friend Anne Marie; his grandchildren, Shane, Gregory and Haley; his great grandchild Elias; and his grandchildren, Olivia, Adelaide, Abbie-Gail, Roland and Travis.
