He was predeceased by: his parents, Nicholas and Anna; and his siblings, John, Katherine, Bill, Mary, Sophie, Alec, Dan and Steve.
He is survived by: his wife Anita Bernice; his children, Daryl, Cheryl (Ted) and Derek (Katie); his grandchildren, Edward (Dusty), Kristin (Lindsay) and Montana (Tyler); and his great grandchildren, Lincoln, Teddy and Katherine.
The family asks that donations, in lieu of flowers, be made in Sam's name to St. Thomas' Church.
