He was predeceased by: his wife Rose Kelly; and his parents, Richard Cook and Anne Cook. He is survived by: his children, Paul, Kelly and Patrick Cook; and his grandchildren, Angelina and Arianna. He is also survived by many nieces, nephews, and friends.
