He was predeceased by: his parents, Rebecca Cook (Rhea) and John Cook. He is survived by: his wife Donna Cook (Jacobson); his daughter Suzanne Lechlitner (Frank Lechlitner); his grandchildren, Barett Hill, Trinaty Hill and Faith Lechlitner; his son Trevor Cook; his sister Bonnie (John); and his nieces, Lisa (Curt), Robin (Dave) and Jenny (Chuck).
