He is survived by: his wife Samantha Bonaccorso; his siblings, Wayne (Judy), John (Doris), Arnie, Leon, Terry (Patti), Bobby, Rick, Sid (Brenda), Randy, Mark and Sherry; and his stepchildren, Zauii, Savannah (David), Richard Kozak Jr. Jr. (Shannon) and Zachary Kozak (Summer).
