He was predeceased by: his parents, George and Anna; his siblings, Olga, Steve, Jean and Wally; and his former wife Dorothy.
He is survived by: his wife Mary Lenore; his daughters, Dr. Cheryl Forchuk, Marsha Skrypuch (Orest), Georgia Forchuk (Neal) and Lara Forchuk (Kevin); his grandchildren, Ian (Maria), Robin, Neil, Callista, Kylie, Lane, Fiona and Brigitte; and his goddaughter Patti.
