He is survived by: his sons, Stephen of Regina, David of Toronto, Nathan (Nicole) of Brockville and Justin of Perth; his grandchildren, Jada and Carter; his siblings, Kevin (Sam) and Kerry (Howard); Marilyn. He is also survived by many nieces, nephews and cousins.

In memory of Jerry donations to CHEO, Loaves and Fishes or the Brockville District Hospital Foundation Palliative Care would be gratefully appreciated by the family.
