He was predeceased by : his siblings, Corrie Wallace, Henk Zeilstra, Tony Van Den Assem, Piet Lazet and Kees Kuiper.

He is survived by : his spouse Joan Van Den (Lazet); his children, Dylan, Mary-Ann Van Den, Oscar, Elizabeth Van Den Assem, Rebecca and Jim Siertsema; his siblings, Margaret Jongsma, Ann Reinsma, Alvin Wallace, Lee Van Den Assem, Ben Van Den Assem and Catharine Cornish; and his stepsiblings, Rita Lazet, Mary Guetter, Margaret Kuiper and Marilyn Lazet.

In lieu of flowers memorial donations to the Christian Blind Mission or voice of the Martyrs would be appreciated as expressions of sympathy.
