She was predeceased by: her parents, Maria Berlowski (Stepien) and Boleslaw Berlowski; her child Charlie Formosa; and her sisters, Joanne and Alice.
She is survived by: her husband Alfred Formosa; her children, Mark Formosa (Mary Jo), Diane Verschraegen and Veronica Formosa (Matt Garrod); her grandchildren, Mark Jr., Geoffrey, Adrian, Ryan, Stacey, Kiana, Keeva, Matthew and Kalin; her great great grandchild Elyse; and her daughter-in-law Donna Formosa. She is also survived by many nieces and nephews.
