With heavy hearts, we announce the death of Jacob Van Oosterom (Chatham, Ontario), who passed away on October 6, 2020. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Simon Oosterom and Jannetje Van Oosterom; and his brothers, Peter and Rick Van Oosterom. He is survived by: his significant other Elizabeth Crombeen; his daughters, Kimberly Dawn (Andrew) and Sheila Horhota; and his siblings, Keith Van Oosterom and Margaret Vanderzweerde. He is also survived by extended family and friends.
