He is survived by: his parent Mary Kosik; his sibling Elizabeth Stubberfield (Betty); his children, Jim (Louise) of Chatham, Cheryl Brooks (Garry) of Chatham, Cathy Ouimet (Marc) of Chatham, Dan (Laura) of Blenheim and John (Michelle) of Chatham; his siblings-in-law, Jim Duchene (Loretta Robertson) of Chatham and Sharon Duchene (late Leonard) of Wallaceburg; and also, Lynn Foundation and Stroke Foundation.

Friends planning an expression of remembrance are asked to consider the ALS Society of Windsor-Essex in honour of John's wife Lynn and the Heart and Stroke Foundation or charity of choice.
