He was predeceased by: his former wife Susan Middleton; and his son Robert. He is survived by: his spouse CJ Lazenby; his children, Anthony Middleton (Kelly), Piper, Paisley, Harper, Rylan, Kyler, Zayne and Jamie Middleton (Janis); his grandchildren, Isabelle and Madelynn; his stepdaughter Bobbie Stoddart (Jeff); his stepgrandchildren, Kyle (Allysin) and Jade (D. J.); his stepgreat-grandchildren, Kemper and Milo; and his siblings, Kevin Middleton and Chris Middleton. He is also survived by his extended family and his wonderful friends.
In honouring Tony's life and legacy and in lieu of flowers, donations can be made in his memory to SickKids Foundation or Make a Wish Canada.
