He was predeceased by: his wife Donna; and his sister Darlene (Paul) of Hawaii. He is survived by: his daughter Nicole; his parents, Harold Cook and Doris Cook of Etobicoke; his siblings-in-law, Diane and Wayne Reid of Baysville; and his nephews, Andrew (Donnabel) and Sean (Miriam).

Donations in Doug's honour can be made to Matthew's House Hospice in Alliston or Teen Ranch in Caledon.
