He is survived by: his children, Kim, Scott (Kim), Steven and David (Amanda); his grandchildren, Erin, Lauren, Derek, Grant, Michael, Brittney, Shelby and Jackson; his great grandchildren, Aubrey and Jack; and his caregivers, Jennifer, Jeremy, Joy, Al and Karen.
