He was predeceased by: his parents, John Lively and Genevieve Lively; his brother Wayne Lively; and his parent-in-law Tudorache Constantin. He is survived by: his significant other Maria; his children, Christina Cordeiro (Nelson), Steven, Amanda Pimentel (Mark) and Nicholas; his grandchildren, Abigail, Isabel, Mila, Ethan and Mateo; his siblings, Rob Lively (Cynthia), Fred Lively (Charmaine), Raymond Lively and Ruby Lively; and his siblings-in-law, Johnny Constantin, Lina Ranieri, George Constantin and Steve Constantin (Jaynne). He is also survived by family from all over Canada, many friends.
In lieu of flowers, donations to the Ontario Heart and Stroke Foundation would be appreciated by the family.
