He is survived by: his mother Paula Collins; his brother Kurtis; his sister-in-law Kristi; his nieces and nephews, Landin, Emilee and Kinslee; his uncles, John, Gilbert Collins and Ken Eady; his aunt Carol; his cousins, Shawn Eady, Shannon Wilson and Matthew Collins; his wife Kristina; his stepchildren, Gabriel, Keelan and Brysen Hayes; his daughter Ashley; and the mother of his child Katie.
