He was predeceased by: his parents, Jacob Hofstee and Adriaantje Hofstee; and his brother Anthonie (Anna). He is survived by: his spouse Lorelle "Lorie" Bell; his children, Kristin Hofstee (Sebastian Ohm) and Kevin Hofstee (Jennifer Gerber); his grandchildren, Dexter, Valerie, Jordan, Penelope and Alexandra; his brothers, John (late Beatrix) and Arie (Jeanne); his sister-in-law Sharon Bell (Neal Thomas); his nephew William; his niece Catherine Bell; and his close friend Maurice Hebert.

For those who wish, donations in Jake's memory may be made to the Glenburnie United Church or to UHKF (directed to Hematology Department at KGH).
