He was predeceased by: his granddaughter Jessica. He is survived by: his wife Suzanne; his children, Jack Jr. (Kerri), Terry (Sherry) and Tracey (Trevor); and his grandchildren, Leah (Derek), Lacey (John) and Jacob (Courtney).
