He is survived by: his wife Jennifer (Middleton); his parents, Jean-Guy Fortin and Clarice Guitard; his sister Melissa (Middleton); his brother-in-law Michael Li; his nephews, Dylan and Nathan; his parents-in-law, Janet Williams (Gordon Banks) and Rod (Janet Middleton); and his extended family, Carol (Williams) and Stu Levine.
