He is survived by: his spouse Diane (Murch); his children, Jack Jr. (Ruth), Perry (Lori), Scott (Joelle) and Mark (Laurie); his stepchild Terry Hardwick (Heidi); his grandchildren, Cody, Colton, Chasen, Nash, Zoe, Curtis, Courtney, Melissa, Trevor, Brett, Clayton and Hunter; his siblings, Wayne (Mary), Martin (Bonnie) and Terry; and his aunt Daisy Thornton. He is also survived by many nieces, nephews & cousins.
