He was predeceased by: his son Forrest Michael; his brother Frank; and his brother-in-law Jerry White. He is survived by: his spouse Shirley Mae Askew (Forrest); his sons, Mark (Brenda) and Douglas; his grandchildren, Christopher (Shannon), Derek (Elizabeth), Amy (Ben) and Morgan; his great grandchildren, Paige, Ben, Owen, Layla, Lucy and Hendrix; his sister Mary; his sister-in-law Barbara; his niece Anne (Perry); and his nephews, David (Meri), Jeffrey (Kerri), Norman (Rhonda) and Andrew.
