He was predeceased by: his parents, Hedley Blake and Adelaide Blake; his sister Jane McCarthy; and his brother-in-law John McCarthy. He is survived by: his wife Patricia (Farrell); his daughters, Theresa Bradley (Tim) and Kelly Decaluwe (Dan); and his grandchildren, Nicole (Ash), Christian (Rachel), Patrick (Libertad) and Benjamin. He is also survived by several cousins, nephews and nieces.
In lieu of flowers, memorial donations made to the Alzheimer's Society Southwest Partners would be greatly appreciated.
