He is survived by: his wife Adriana; his sons, Steven (Elisa) and George (Toni); his grandchildren, Kyle, Brianna, Madison, Brayden, Klara, Brooklyn and Avery; and his great grandchild Rilynn.
In lieu of flowers, the family asks for charitable donations be considered to the Milton District Hospital Foundation in honour of Jack.
