He is survived by: his significant other Pasqualina "Carmela" Fazio (Costabile); his siblings, Lina Rizzo (Mario) and Franco Fazio (Pina); his grandchildren, Jordan Fazio (Carly) and Julia Fazio (Matt); and his siblings-in-law, Armando Costabile (Donata), Theresa Pellegrino (Frank), Frank Costabile (Fulvia), Fred Costabile (Vivian) and Gaspare Costabile (Olga).
