He was predeceased by: his parents, Walter Cook and Audrey Cook of Hawkstone. He is survived by: his spouse Lorie; his sons, Adam and Troy Cook of Oro Station; his siblings, Gary (Debbie) of Oro-Medonte, Linda Coote (Keith) of Oro-Medonte and Steven (Wendy) of Oro-Medonte; his mother-in-law Marjorie McQuay of Barrie; and his sisters-in-law, Nancy Keene of Barrie, Sandra Olver (Randy Quiring) of Drumheller Alberta and Cathy (Barry Graham) of Peterborough. He is also survived by Uncle Ronnie to14 nieces and nephews and 17 great nieces and nephews.

In lieu of flowers donation may be made to The Hudson Regional Cancer Centre, Barrie or to Mariposa Hospice House Orillia.
