He was predeceased by: his parents, Arthur Middlestadt and Helena Middlestadt; and his siblings, Vern Middlestadt (late Vicki), Beatrice Horricks (late Bob), Helen Byrne (late Jim) and Eileen Armstrong (late Don).

He is survived by: his spouse Shirley Middlestadt; his children, Kim Middlestadt of Pembroke, Kelly O'Grady of Pembroke, Scott Middlestadt (Donna Sadler) of Pembroke and Tiana Knox (Steven) of Pembroke; his son-in-law Mike O'Grady; his grandchildren, Aubyn, Ayron (Codie), Hannah, Donal, Arthur, Miranda (Tyler), Max, Chez (Billy) and Chanelle; and his great grandchildren, Bryce, Brock and River. He is also survived by many nieces and nephews.
