He was predeceased by: his parents, Emily and John; and his brothers, Ken, Doug and Don.

He is survived by: his wife June Kent; his sons, Rick (Laurie), Greg (Bonnie), Steve (Diane) and Barry (Sandra); his grandchildren, Lindsey, Jordan (Elena), Kortney (Tyler), Kelly, Robyn (Robert), Ryan, Chelsea (Michael), Brayden (Nicole), Abigail and Jacob; his great grandchildren, Colton, Hunter, Jack and Patrick; his siblings, Vern (Marilyn) and Carol (Don); and his siblings-in-law, Leo, Anne (Bill), Marie (Jim) and Frank (Imelda).

In lieu of flowers, donations in memory of Jake can be made to the Alzheimer's society of Durham Region.
