He was predeceased by: his parents, Mary Hazlitt and Harold Hazlitt; his stepfather Walter Middlebrook; and his stepbrothers, Robert Middlebrook and Bruce Brigham.
He is survived by: his wife Peggy; his sons, Jay Hazlitt (Kerri) and Mark Hazlitt (Kris); and his grandchildren, Katelyn, Kristen, Sydney and Callie.
