He was predeceased by: his wife Theresa Lewis; his son John Hugh Lewis Jr.; his grandson Jason; his brother David Lewis (late Audrey); and his parents, Joshua Lewis and Doris Lewis (Cook).

He is survived by: his children, Susan Heatherly (David), David Lewis (Erdean) and Philip Lewis (late Michele); his grandchildren, Daniel, Lindsay, Ryan, Brandon, Corey, Joshua and Lauren; his great grandchildren, Logan, Cole, Emma, Cayson and Finnley; and his brother Richard Lewis (Beth). He is also survived by several nieces and nephews.
