He was predeceased by: his parents, Gilbert Collins and Jean Collins; and his wife Bonnie Collins. He is survived by: his children, Mark, Lisa Collins, Matthew, Marisa Collins, Meredith and Parry Cadotte; and his grandchildren, Aidan, Xander and Ethan.
Donations made in memory of Jack to the Diabetes Canada would be appreciated by the family.
